Can someone explain why items on checklists aren't always confirmed by both pilots? Some actions are called by the PNF and the PF checks the item and responds, where as other items are sometimes actioned and checked by the PNF.

For example (747-400 Normal Checklist)

PNF: Flaps
PF: 20, 20, and GREEN
PNF: 20, 20, and GREEN

or

PNF: Speedbrakes
PF: ARMED

Those important items that need to be checked by both PF and PM(PNF) have a response requirement by both. Those items that only require confirmation by one pilot, are read by PM and responded to by PF.

Thanks for that, makes my question seem rather stupid. :\ So it depends on "how important" a particular item is on the checklist.

Far from stupid! Some operators actually have some check-lists that are read and responded to by PNF. Of course, 'airmanship' would suggest that the other pilot will 'cast an eye' at what is being recited, but pressurisation checks after take-off (ref Helios?) was one of those 'unchecked' checks in some airlines - although I suspect there are a lot more 'checks' being done now.:ok:
