LA has had flights between LAX and a couple of its suburban airports, I believe for quite a while. The traffic problems in LA are legendary, and well documented.

Have also just noticed flights between BZE (10 miles NW) and TZA (1 mile N) in Belize City. Looks like a lot of short hopping out to the diving islands and other coastal resorts, but how does TZA stay viable?

Are there any other cases of commercial (fixed wing) flights operating between two airports under the same designator code?

There's a number of LHR-STN flights being operated at the moment. Although you can't book them ... :D
