Safari Helicopters Lose One in Hawaii

Heard that Safari Helicopters lost an AS350 in an accident in Hawaii. Not sure what happened but rumour has it collective not locked down doing hyd off check, but again this is only rumour. Unless anyone else has some up to date info.
