Tornado blue on blue with Patriot BOI.

A terrible event.
BOI says lots of small errors, IFF failed but crew not aware, plus patriot system not plugged into rest of command and control.
Fair comment, or could they have been more critical of US? Apparently it came down to a young female operator not getting a reply on her radio in time...
