Minister's pilot killed in plane mishap

Dehra Dun: In a freak mishap, the pilot of the helicopter in which Union Agriculture Minister Rajnath Singh was on election campaign, died after his head was chopped off by its rotor in Uttaranchal.
Col. Anil Nakra, on seeing some smoke emanating from the helicopter, went to its rear to see the fault and his head got chopped off by the revolving rotor, Senior Superintendent of Police S S Negi said.The incident took place when Singh was addressing an election rally at Jasra in Udham Singh Nagar.

The Minister skipped his rally at Saharanpur and proceeded to Delhi by road. The SSP said an inquiry has been ordered into the incident.

This tragic incident brings the question of what affects our abilty to concentrate? Fatigue is often discussed, but how do we maintain the ability not to have our thoughts diverted? Training plays a great part. This man died in what appears to be an act of stupidity - but why?
