Apart from Bmi, KLM and Lufthansa who else does Manchester to Hamburg ? Are there any low cost operators ?

The only real lo-co option would be Germanwings to CGN, but you still then have a 4 hour train ride.
You could also consider EX from LPl to SXF - then about 2 1/2 hours once you get to the city centre.
Otherwise, as you say, take your pick with the scheduled airlines.
Yes, HAM is a nice airport, have never been there but it does feature in the rather lame Bond movie "Tomorrow Never Dies"
