The authors reference to plan continuation bias -“the deep-rooted tendency of individuals to continue their original plan of action even when changing circumstances require a new plan” - is a sobering one given current external (and internal) pressures on aircrew. The problem is the more someone ‘gets away with it’ the higher their risk threshold becomes until either outside intervention (inclusive of other crew) or an outcome like this.

Hopefully not a tragedy in vain.
