I don't think there is a consent form before taking the test but what I hear from different people is that you cannot take another cut-e test again , whether it was from cathay or dragon, after a year you've taken it. Like you said, they are ultimately the same company and would bound to share the same info. . Sucks that it has to be this way because back then if you fail one of them, you could still try the other.
