If memory serves me correctly, ICAO Annexe 14 is the definitive document about CFR categories and many other things, but it's well set out in CAP 168 as already mentioned.

I just looked it up -- ICAO sell Annexe 14 for $129. Your nearest airport should have a copy in its library..."should" is the operative word.
