HotDog, I think Atacks was referring to the A330 crash which killed Nicholas Warner (AB chief test pilot) and all on board.
However it was an oversimplification - While mode-confusion did play a part (AP in altitude acquire mode caused excessive pitch up while low and slow), the accident was also due to PF over-rotating and PNF (Warner) being distracted by preparing the test conditions for the next approach.
It was this incident that got AB to change their tune on certain aspects of their system design (multi-function knobs being a no-no, for example).
