If you want a collectivist health care system that means everyone is in it not just those who are more cost effective. The alternative is to abolish Medicare and go to a health insurance system where you get the health care you can afford.
The smokers and drinkers pay a LOT of tax on their addictions.
An equivalent could be to charge the anti-vaxxers much higher medicare levy to reflect the risk that they could take up an ICU be for a few weeks, or better still present them, or thier estate of they die, with the bill..
