Also from Spain here and I thought you could request the certificate in English? At least one of the things you were required to specify when asking for it is in regards to the country you need this for, and I was assuming that involved translation into that language? (I’m still waiting for the certificate to come). I also believe translation of the documents is required (other certificates you can actually get those directly in English, eg the University degree), but right now I don't remember if I had read it somewhere or if it was just my mind playing tricks.

Regarding the form, just leave that cell blank. It specifies there something like "new entries will be provided with a number after receiving the form", meaning you don't have one when submitting this.
