DGCA CPL conversion - performas for checks/reports

Hey,
Would appreciate if anyone could share the performas for the checks for foreign license conversion to DGCA. Like the performas for x ctry checks, day/night/ir checks etc...
Basically looking for some paperwork from the tonnes thats gotta be submitted.

Thanks preetham, I am aware of that flow chart.

What I am looking for is the performa for the certificate that has to be issued for each check.
