No more visual approaches at night.

FAA says no more visual approaches to 28R when 28L is closed. And two Controllers in twr until evening rush over.

To accuse the crew of flying fatigued is serious stuff today. And for Aircanada.

CVR lost!

It is a shame the CVR can not be recovered as it is essential to understand what happened.
The crew has conveniently developed amnesia.
Thinking that 28L was still active when it was on the NOTAM...

CVR data is lost!

I just read the NTSB official info. It did not mention the CVR. The FDR read out was provided by Canadian TSB, ergo I dare conclude the CVR was not secured.
They would have mentioned it.

There is no excuse whatsoever for what these two chaps did and they better have preserved the CVR.
We need it to understand how it was possible and prevent other incompetent crew doing the same....

Norwegian Chief Pilot just commented on this incident and stated that a visual night approach is prohibited in his company.
This is the case in most EU companys.

Visual at night is not a good idea without a ILS...

Visual at night is not a good idea without a ILS or VNAV- PATH as primary and visual as backup.
This near disaster proves it.
