It is urgent to get this report out as it will also reveal a large amount of recommendations for this...

Black dot for light house!

I just pulled out my Dads old ICAO 1:500 000 VFR chart to check how a lighthouse is marked.:
It is with a black dot, no elevation or height and Feiestein Light ( WestSW of SVG for example)

SAR overwater letdowns are safe, as I understand.
Problem here is modern equipment paired with homemade charts, SOP etc.
Lots of details indeed, but no quality control or watertight or foolproof...

I grew up with Jeppy plates and like them, now we have some other jazz that I that is not so good.

Blackrock Sling vs Mayo. 80feet vs 300?

Remote contributing factor?
Could the crew have confused the height of Slingo Blacrock Lighthouse ( 80feet) and Mayo by asosiation. Not likely, but you never know.

Instrument Approach error?
