re 'Starting both engines' some pilots have forgotten to do that rather annoyingly for them
perhaps a button that says "start both engines" would help?
"gross error check" is the best antidote to 'superstition'

to the belief in an engineering solution that is also not based on reason, it is on topic.
if there is no credible evidence that an engineering solution works yet people belief in it, then it qualifies as Superstition which seems alive and well here..]

As for your views on the Mods Point of View regarding twin engined being other than superstition as defined, you are entitled to have your view but it appears I am not entitled to have mine. The theory supporting it has been discredited (10^-5 x 10^-5, really is nonsense!)

I have heard of at least one example in a helicopter and I should think that others on this forum will recall this mistake

This is your latest excuse for pushing the single is best line is it? If you don't have a second engine you can't forget to start it - dear oh dear.
The difference in a helo is you will discover the second one isn't running when either a. you do your checks properly or b. try to lift to the hover.
In the example you linked to they didn't get airborne but aborted the take off run.
And no, I haven't ever forgotten to start the second engine, even when I was teaching on both singles and twins on the same day.
You do need to get over yourself and move on from this pointless crusade.
Back to the superstitious thread - I always used to fly with lifejacket number 13 whenever it was available - many pilots wouldn't touch it.

If we're really lucky, ANFI might just shut up about his ridiculous campaign about the number of engines on aircraft. But no luck so far. Maybe we should have a whip round and buy him a twin rating.
AnFI will quote some statistics that show the risk of one superstition failing is actually very low so there is absolutely no need for a second superstition at all.
He will then blame the superstition manufacturers for skewing the stats and making us use two superstitions when only one is needed to ensure safety
