UK CAA Form 1158

I would like to ask a question about the requirements of the filling of this UK CAA form.

I am about the revalidate my type rating and I also started to work in a new company.

As a conversion training for my new company I was required to complete 4 simulator trainings and a proficiency check. I finished the 4 training sessions and I will do my proficiency check tomorrow. I just realized that in the form 1158 there is also a practical training section which should have been filled by the instructor who released me for the proficiency check. But the last instructor only completed company forms to release me for the proficiency check.

Somebody told me not to worry about it because it shoud only be filled if you are doing a initial training for the type. Should I follow up the last instructor and get it filled ? If someone has an information I will be grateful, thanks
