Arabic translation help needed

not sure where to put this request.

I have recently returned from Dubai and appear to have been fined by Dubai Police for a traffic violation in my hire car. I am intrigued as to what I have done but the document is in Arabic

I would be very grateful if someone with arabic reading skills could help, please PM me, so I can send a jpeg of the form.

From what I hear it was pimping and kerb crawling without the adequet lisences and or training from the authority but this is just a rumour

just go to bur dubai police along the sheik zayed rd,you see it on your left when you drive in direction of mall of the emirates and it is situated just before the mall at around 500m.probably 200 or 400 dhs fine.
