Reporting Error In Aircraft Maintenance

I am doing a Masters and as the final part of this I have to complete a thesis. The subject is Reporting Error in Aircraft Maintenance, and the purpose of the questionnaire is to find out if maintenance personnel are reporting and if not why not.

After 15+ years in maintenance myself with the opinion I have extremely high integrity it is still a question I struggle to answer, why is reporting so difficult for maintenance personnel? The questionnaire responses will hopefully determine some possibilities that may be the reason behind non-reporting:

- error awareness and training - do we know what to report or what is expected of us to report
- reporting system design - easy to use, relevant questioning, amount of information to be supplied, accessibility
- company attitude & commitment
- engineers' personality - do we just have the head a problem is to be solved and not procrastinated over

Below is a link to a questionnaire, it takes around 5 minutes to complete. Your help and support in completing the questionnaire would be greatly appreciated and would assure the success of my thesis.

Its completely anonymous, absolutely no way of knowing who has completed it or not and there are no questions asking about what company you work for, etc.
