Pilot Error ?

There is an element of truth to the above. I know on multi sector days your brain can almost go into autopilot mode itself. Not that you are not functioning correctly but in some ways a more relaxed attitude and relaxed to everything
In some ways you can connect it to the second wind on running a marathon that getting to the point that you cannot run any further you go through that into something else more like running on automation
I had that on a Europe USA ferry which involved a very long day arriving in Dallas late at night.

Ah, The Impossible Choice
Sure! IF it cannot be blamed on something else, it has to be Pilot Error, so they say. Nuts.
Every walk-around misses something. Is that Pilot Error? Not eve bit of electronic can be fully tested before each segment. Is that an error? On the flip side, continuing with flight, failing to return to ground at the first post-QHR check IS a major error and the cause of many major events. If it don't fly right, ,get the SOB back on solid ground, ASAP. I cannot imagine a case where that is a wrong choice, at least from the safety perspective. Far too many still wish to be bold, thinking, "I can deal with anything..." Don't do it; LAND the SOB and you will live far longer - and I won't have to read as many reports about dead pilots. That becomes old very quickly. -NFZ

Most accidents are a succession of errors and yes that can start in planning or the walk around or even a row with the wife.
Some like flying into a mountain covered in cloud is obvious pilot error but aircraft failures??? There is an argument that flying is easy and its when there are problems that we earn our money as pilots.
A lot of training is learning to deal with aircraft failures and in some ways we are expected to deal with those.
In that sense failure to deal with aircraft failures is also pilot error
