What are your current qualifications (ratings, etc.) and when was the last time you flew or wrote a written examination from Transport Canada?

Your ROC-A is valid for life.

The medical certificate validity periods are in CAR 404.04(6) (http://laws-lois.justice.gc.ca/eng/regulations/sor-96-433/page-51.html#h-414) (also p 15 of your booklet).

The validity period in Canada for language proficiency at ICAO levels 4 and 5 ("operational") is five years (AC 400-002 (https://www.tc.gc.ca/media/documents/ca-opssvs/400-002-eng.pdf) refers).
