Go to the ATSB site, uncheck the final reports box and the completed investigations box. There are 98 pending investigations. The oldest is the Bankstown / Mojave accident which occurred on 15 June 2010. 2 years and 5 months ago.

If the purpose of investigations is to learn from an accident and use the information to help prevent future accidents, then isn't this system completely failing?

Many of the open investigations are issues like "Breakdown of separation assurance" where there is nothing physical to examine, only a very small number of people to interview. Why does it take so long? Aren't we actively being done a dis-service by the ATSB by the delay?

The malaise has been in there for a long time now. To know the guts of the problem you'd have to have inside information. An in depth study of the workings would need an investigative journo with the skills of a Chris Masters.

If you knew where these roosters drank, if they do, then that could be a start.
Methinks.

one could suspect -

lack of resources, and

no measures being applied for timely delivery of reports.

It’s merely one example of a widespread malaise.

The pollies want government agencies to do more and more with less and less.

No one in a government agency is allowed to deliver bad news to the pollies: only ‘options’ and ‘announceables’.

The inexorable and inevitable consequence is that government agencies become increasingly incapable of dealing with the workload, and ‘led’ by people who are prepared to pretend there’s no problem.

Yep, agree. :ok:

But the Dolan 4C interview and the Senate Inquiry seemed to demonstrate that the ATSB is 'led' by people who haven't the faintest bloody idea that there IS a problem, or WHY there's a problem! :(

Private industry is expected to make productivity improvements each and every year. Anyone with a long term supply agreement with a car company will have it entrenched in a contract. Why should we not expect that the public service should do the same?
