RANCHI: A helicopter carrying chief minister, Arjun Munda, wife Meera and three others crash landed at the Birsa Munda airport around 12.25 pm today. All on board the chopper are safe but have sustained injuries for which they are being treated at the Apollo hospital near here.

A "satisfactory" outcome for those on board at least.

I scan-read lots of available reports and watched related online videos on this incident (at least 15 in this case as it is high profile in India and well reported). Looks like it was a 109C with a tail rotor issue and a fast landing made on wheels with the aircraft then turning over and rolling a few times. Sort of reminiscent of the Met 222 all those years ago at Hatfield with the American VIPs on board...

Curious how these Indian 'incidents' and accidents all seem to occur whilst carrying Government officials.

Government officials are pretty much the only people who fly over there.
You get the occasional actor and cricket player, though.

What's 'curious' is the rate and seriousness of engineering/maintenance related accidents and IMC/CFIT related pilot 'error' in India. Having read the RWSI list of accidents last year.

An A109 suffered a jammed Pedal with a VIP onboard in India. The Pilot was very experienced on type but the aircraft rolled over. As per the news, the padels jammed enroute to a helipad. Indicating a left pedal lock. The pilots turned back to the airport and tried 4-5 approaches before deciding to put the aircraft on the runway.
What in your opinion went wrong, what are possible causes that might lead to a padel jam?
What did the pilot miss out on handling this emergency that could have prevented rollover?
Tips and trick highly appreciated!

He probably forgot to stick his leg out of the door after landing to fend off the ground, which has been shown to prevent dynamic rollover on Brantley helicopters.

P.S. I saw a crashed car on the motorway yesterday. Anyone got any idea what caused it?

Was that a front or rear padel that indicated locked?:suspect:

Indian examiners do not do stuck pedals on check rides. They don't do real autos neither, collective down only.

Neither do the FAA after they ended up paying for this one.....you may recognize it from the movie "Jurassic Park"...

tottigol, there are people in the world who want to learn and share something called "knowledge". And in the field we both are, its advisable to learn from others' mistakes than learning it in the hard way.

I have got some more information on the case. According to various sources the cause was that the "Duplex Bearing" came out of the casing in flight because the "Ring nut" gave away. Once the Duplex bearing is out of place we lose all the control on the tail rotor pitch control.

However the reason as to why it happened is still uncertain.

Please view the attached figure. Generally once the Duplex bearing is set in place, the Ring Nut is tightened to hold the bearing at its place and this ring nut is further secured by two lock wires.

Why did the ring nut 'give way' ?? Even when the wire locking was done ??Was the ring nut and the other threaded parts in good condition ??
Was the wire lock done correctly ??
What do the AW guys have to say ??
Still does not sound convincing that the Pilots could not pull off a better landing !!

According to the manufacturer, there has been no report of a stuck pedal incident in an A109A.

As of 2004 I suppose
