I remember about 25 yrs ago I was instructing in a Decathlon and the student on the right hand seat went quiet, leant to the side and would not answer my questions (intercom).

The radio was only reachable from the front as were mixture etc.

I called "flight service" ( back then )and requested an ambulance meetus on our return.

Ended up the guy worked in a saw mill and was regularly bitten by various insects and bugs and just prior to take off he had been bitten by something. He wasn't concerned and hadn't told me. The Dr suggested that it was most likely a build up of toxins over time.

Makes you think though, hell of a lot of single pilot aircraft out there with relatively few incident due to incapacitation.
