Third (excluding several hard landings). An aircraft being delivered to the Jharkhand Government went down in November 2005 (http://www.indianexpress.com/oldStory/82743), and the IAF's Sarang display team lost one in February 2007 (http://www.thehindu.com/2007/02/03/stories/2007020321970300.htm).

When looking at the crash video and pictures, and no serious injuries, it seems that the Druvh has a pretty strong (crashworthy) cockpit design.

Dhruv seems to be a great machine looking at the performance, the most recent landings at 19480feet AMSL in the Himalayas and of course the maneuvers it can do. I can't wait to find out what went wrong here.
Another news video at youtube shows how the aircraft was banked to the left during the decent. They are also reporting fire coming out of the rear.
