ARN Cost?

I had a look on the CASA website and the form to apply for an ARN but I can't seem to locate any cost.

Can anyone here clarify what the cost is or is it just simply a matter of posting the form away?

one more question if you dont mind..

If I write Bob as my given name and Alex as my Other Name..

Do I always have to wrote Bob as my given name in future forms
or can I switch it to Alex at a later date?

Bob Alex xXmuffin0manXx, I have no idea. I suggest a compromise, perhaps Boex or Alob.

Write whatever your legal name is, or ask CASA.
