Pay office phone number and address for tax credits purpose....

Trying to fill out my tax credits form and it requires my employers pay office address and phone number...i can't lay my hand on any P60's at the mo...does anybody have the address and a phone number? :\:\

Anyone....is it just the JPAC phone number I need to give them....
