I don't see why you shouldn't have to pay for a sick note. Particularly if you actually get to see the Doctor. Usually by the time you see the Doc the cold or hangover (whatever)
is gone and you are simply cluttering up the waiting room.
I know of one or two places where sick notes are pre signed by the Doctor and handed out by the receptionist on provision of the usual fee.
